Information from state Health Department records for inspections April 14 through May 1. Restaurants are listed in alphabetical order. All reports are from regular food service / food store inspections unless otherwise noted.

Critical violations are defined as items that relate directly to factors that lead to food-borne illness, and must be corrected immediately.

Noncritical violations are defined as items that relate to maintenance of food operations and cleanliness.

April 14 Maria's 2503 S. Walton, Bentonville Critical violations include: Do not store raw eggs above pre-cooked foods. Prep cooler is not keeping taco condiments cold; front door is not attached. Unit must be replaced. Maintain cold foods at 41 degrees or below at all times. Repair fajita cooler to maintain food at 41 degrees or below.

Noncritical violations include: Need thermometer in fajita cooler. Cooler reads 57 degrees.

April 18 Siloam Springs Riding Club Rodeo Grounds, Siloam Springs Critical violations include: Reheat hot-dogs before putting in hot holding. Need test strips for sanitizer.

April 23 Sonic 115 S. Bloomington, Lowell Critical violations include: Remind employees to wash their hands, not just sanitize, corrected. Employee cleaned in-place equipment and floor with a spray hose and spray from floor can contaminate already clean utensils. Rewash and sanitize all equipment that could have become contaminated. Care should be taken to prevent contamination of equipment during cleaning. Equipment must be sanitized. Sanitizer exceeds 400 ppm. Quaternary ammonia concentration should be 200 ppm. Do not store ham above the pan load line.

Noncritical violations include: Replace broken and cracked containers.

Domino's Pizza 105 S. Dixieland, Lowell Critical violations include: Sanitizer should be 200 ppm.

Dairy Queen 119 S. Bloomington, Lowell Critical violations include: Maintain cooler so food stays at 41 degrees or below.

April 24 La Hacienda 2010 S. E. Main, Siloam Springs Critical violations include: Employee drinks need a lid and straw in prep areas, corrected. Raw meats when placed vertically on shelving should be placed fish, beef, then chicken from top to bottom, corrected.

Noncritical violations include: All restrooms need self-closing doors.

Tower BBQ 110 Progress, Siloam Springs Noncritical violations include: Coving needed in washroom / cashier area.

Smokin Joe's 1204 S Walton, Bentonville Critical violations include: Maintain sliced and pulled meats at 41 degrees or below.

Maria's Restaurant No. 2 2813 W. Walnut, Bentonville Critical violations include: Cool food from 135 degrees to 70 degrees in two hours. From 70 degrees to 41 degrees in four hours.

Noncritical violations include: Need a thermometer to check internal temperature of food. Repair floor where tiles are missing. Clean all walls and equipment including your dishwasher.

The following is from the Dogwood Craft Festival April 25 Billy Jack's Rootbeer Critical violations include: Sanitizer should be between 50-100 ppm for chlorine.

L and Mini Melts Critical violations include: Sanitizer concentration should be between 50-100 ppm.

Louisiana on Wheels Critical violations include: Sanitizer solution must be 50-100 ppm chlorine to be effective at sanitizing;. Need new bottle of bleach, use test strips.

Project Graduation 22977 Lawlis Rd, Siloam Springs Critical violations include: Must wash, rinse and sanitize as a three-step process to clean food contact surfaces. Must use proper concentration and test sanitizing solution with testing strips; must be 50-100 ppm for chlorine solution.

Top Chef Concession Critical violations include: Must wash, rinse, sanitize all dishes, utensils, food contact surfaces. Must properly use three step process in three compartment sink, corrected. Must have sanitizing solution to sanitize food contact surfaces and to store sanitizing wipes at a concentration of 50-100 ppm chlorine or 200 ppm QUAT, corrected. Must store all foods 6 inches or above off of floor to prevent contamination of foods, corrected. Must keep all hot foods at 135 degrees or above at all times, corrected. Must keep all cold foods 41 degrees or below at all time corrected.

Noncritical violations include: Must have accurate and operational thermometers in all cooling units. Must have test strips to test concentration of sanitizing solution and ensure use of strips every time you prepare solution.

Don's Catfish Kitchen Critical violations include: Maintain hot food at 135 degrees or above. Establishment will use time as a control. Do not hold food for more than four hours. Food must be destroyed after four hours. Chlorine sanitizer should be between 50-100 ppm.

Noncritical violations include: Double check heat lamp source to ensure bulb is actually for heat lamps.

April 28 Loyd's Little Land 1723 Concord St., Rogers Critical violations include: Employees cannot eat at prep table during break, corrected.

Carbajal Bakery 1501 SE Walton, Bentonville Critical violations include: Employee drinks need to have a lid and straw, corrected. Hand washing sink needs to be supplied and accessible, corrected. Eggs must be refrigerated, corrected. Spray bottles of chemicals need to be labeled, corrected.

Wal-Mart Super Center 406 S. Walton, Bentonville Noncritical violations include: Need a thermometer for meat cooler.

Wal-Mart 100 Sushi Bar 406 S. Walton, Bentonville Critical violations include: Do not place utensils in the hand-washing sink. Hand-washing sink must stay accessible at all times, corrected.

McDonald's 406 S. Walton, Bentonville Critical violations include: Establishment uses time as a control. Items must be marked with time use by or time destroyed or time taken out of temperature control. Accurate time is required. Employee marked time during inspection and it was more than four hours. Employees must wash hands prior to glove use even when changing tasks. No sanitizer at three compartment sink. This is a repeat violation. Use packet sanitizer until repaired. Chlorine 50-100 ppm Quaternary Ammonia 200 ppm. Manger does not know which test strips are for dish sanitizer. All employees must be trained on proper sanitizer. Fryer cleaner was open and above fryers. Employees must wash hands after placing raw hamburgers on grill-prior to making sandwiches.

Noncritical violations include: Employees must wear hair restraints during food prep-even managers.

Building Bridges at BHS 1901 SE J St., Bentonville Critical violations include: Need single-use towels for kitchen hand washing, corrected.

Noncritical violations include: Effective hair restraints required for all food prep staff. Clean interior of refrigerator.

Chen's Garden 428 Town Center, Bella Vista Critical violations include: Vegetables cannot be stored below raw chicken, corrected. Chicken must be less than 41 degrees cold holding, corrected.

Noncritical violations include: Vegetables cannot be stored below dirty dishes, corrected. Wiping cloths need to be stored in sanitizer water. Icecream scoop needs to be stored in water more than 135 degrees or in running water.

ABC Happy Kids Learning Center 301 S. E. 28 th, Bentonville Critical violations include: Spray bottle concentration should be 50-100 ppm. Redo spray bottle.

Noncritical violations include; Raw wood under dishwasher must be finished so that the surfaces are smooth and easily cleanable.

Shell Superstop 1311 S. Walton, Bentonville Critical violations include: Label spray bottles correctly with contents. Do not reuse a bottle for different chemicals.

Noncritical violations include: Clean under milk shelves in cooler.

IHOP 4604 W. Walnut, Rogers Critical violations include: Salad prep cooler is not holding food at 41 degrees or below. Automatic dishwasher not dispensing quite enough sanitizer adjust as necessary. Chlorine 50-100 ppm barley under 50 ppm.

Noncritical violations include: Need a thermometer in salad prep cooler.

April 30 Manna Center 196 E Kenwood, Siloam Springs Critical violations include: Wash, rinse, sanitize all equipment and utensils as well as food contact surfaces. Sanitizer wipes and sprays are not approved for commercial use.

Noncritical violations include: Need test-strips for sanitizer, chlorine 50-100 ppm.

Siloam Springs Senior Center 730 Heritage Ct., Siloam Springs Noncritical violations include: Do not reuse singleuse containers for food storage.

Mazzio's Pizza 906 W Kenwood, Siloam Springs Critical violations include: Automatic dishwasher is not dispensing sanitizer. Maintain cold food at 41 degrees or below.

Noncritical violations include: Establishment must check sanitizer every day. Need thermometers in prep coolers.

LaHuerta 1270-A Hwy. 412, Siloam Springs Critical violations include: Hand-washing sink is for hand washing only. No soap at hand-washing sink, corrected. Do not stack food above the load line. Maintain at 41 degrees or below. Cool food from 135 degrees to 70 degrees in two hours. From 70 degrees to 41 degrees or below in four hours.

Noncritical violations include: Keep containers covered in walk-in. Repair floor as necessary. Do not reuse single use containers for food storage.

Price Cutter 357 103 N. Dixieland, Rogers Noncritical violations include: Meat department found without test strips; must have test strips on site to test chemical sanitizer and ensure proper concentration. Repeat violation Price Cutter 357 - Deli 103 N Dixieland, Rogers Critical violations include: Dirty towels found touching "to-go "serving boxes. Must label potentially hazardous foods with a discard / open date if kept for a period longer than 24 hours, corrected.

Hardee's 1500502 1625 Hwy. 412 E, Siloam Springs Noncritical violations include: Replace insulation guard around milk cooler. Surface should be smooth and easily cleanable. Air gap required for ice machine condensation line.

May 1 El Rincon Catracho 1610 S. 8 th St., Rogers Noncritical violations include: Tighten the employee door so it closes all the way by itself, corrected. Paint the edges of the prep table serving line, corrected.

Siloam Springs High School 1500 W. Jefferson, Siloam Springs Noncritical violations include: Bulk containers of flour and sugar need to be labeled.

Allen Elementary 1900 N. Mt. Olive, Siloam Springs Critical violations include: Frozen ground beef being stored above ready-to-eat foods. Need to be stored below all ready to eat foods or precooked foods to prevent any possible contamination, corrected.

CK Sweets and Treats PO Box 483, Gravette Noncritical violations include: Need test strips for chlorine sanitizer.

Pizza Hut 391 E Hwy. 412, Siloam Springs Critical violations include: Clean out dishwasher; excessive food in dishwasher. Employees must wear gloves when they have a cut or bandage on the hand. Maintain cold foods on salad bar at 41 degrees or below. Monitor temperatures to ensure food stays at 41 degrees or below.

Noncritical violations include: No jewelry on wrist or hands except one plain band.

Taquera El Rancho 715 E. Jefferson, Siloam Springs Critical violations include: Hand-washing sink is for hand washing only, do not wash hands at three compartment Noncritical violations include: If ice is used to keep food cold it must surround the food completely. Hand washing sink is for hand washing only do not wash hands at three-compartment sink. Store silverware handles up to prevent contamination from customers hands. Store scoops handles up in bulk containers.
